This Privacy Policy explains how Premier Neurosurgery collects, uses, and protects information when you visit premierneurosurg.com.
This policy covers the website only. Forms on this site
Our contact form and our physician referral form are hosted by Jotform, a form provider that supports HIPAA compliance requirements and with which we maintain a Business Associate Agreement.
When you fill out either form, your information goes directly to our secure systems. It is not stored on this website, and it is not sent to us by email. Our staff retrieves submissions by signing in to Jotform.
The contact form asks for name,phone and email. The referral form is for referring physicians and collects patient and insurance details needed to process a referral.
Jotform holds submissions for 1 year, after which they are deleted. Information that becomes part of your medical record after we act on a submission is governed by our Notice of Privacy Practices and the retention rules that apply to medical records.
Information collected automatically
When you visit the site, our hosting and security provider records standard technical information, including your IP address, browser type and version, device type, the pages you view, the time of your visit, and the address of the site that referred you.
We use this to keep the site running, protect it from abuse and attacks, and understand general traffic patterns. We do not use it to build a profile of you or to target advertising.
Cookies
This site sets a small number of cookies. Some are set by WordPress to keep the site functioning. Cloudflare, our security and performance provider, sets cookies to tell real visitors from automated traffic. Jotform may set cookies within the form area to keep your entries in place while you complete a form.

How we use your information

We contact you only about the request you made. We do not send marketing email, newsletters, or patient satisfaction surveys, and we do not add you to a mailing list.

- Service providers who help us operate this site and our practice, including Jotform, each of which is required to protect it and use it only to provide services to us

How we protect your information
The site uses SSL encryption, so everything you send through it is encrypted in transit. Form submissions are held by Jotform under its HIPAA-compliant configuration rather than on this website or in ordinary email.
No website or method of transmission is completely secure. If you have something confidential to discuss, calling the office at 925-430-5335 is always an option.
